Inventar percursos, reinventar realidades: doentes, trajectórias sociais e realidades formais

Abstract

A complexidade dos problemas que caracterizam o processo de articular cuidados de saúde impõe uma perspectiva de análise do sistema de saúde radicalmente diferente daquela que é utilizada habitualmente. Neste sentido, optou-se pela realização de um estudo de caso que permitiu uma leitura em profundidade das representações e das práticas que tecem a densa malha social dos processos que os profissionais e os doentes protagonizam para conseguir atingir objectivos e desenvolver estratégias que maleabilizam o sistema de saúde à integração da diversidade das suas culturas. Assim, foram colocados frente a frente uma comunidade com os seus recursos globais e um sistema com as suas instituições legitimadas, como duas ordens sociais igualmente complexas e com fundamentos culturais igualmente diversos. Este estudo permitiu determinar a equação fundamental da resistência do sistema de saúde aos processos de mudança, emergentes nas trajectórias sociais definidas pelos indivíduos nos consumos de saúde e que se vão pronunciando ao longo da análise das representações dos profissionais sobre os cuidados de saúde e nas posições por eles tomadas face à articulação dos cuidados. A construção das representações dos saberes dos doentes e das formas sociais que adquirem no seu accionamento constituíram um instrumento analítico, de valor inestimável, para identificar e interpretar as posições tomadas quanto ao seu reconhecimento e legitimidade. O sistema de saúde ganha visibilidade como um sistema global de resistência ao funcionamento dos saberes profanos e à diversidade de estratégias sociais a que estes saberes dão lugar.The complexity of the problems that characterize the process of articulating health care needs imposes a radically different perspective for the analysis of the health system than has been habitually used. This study opts for a case study method that provides a deep reading of the representations and practices that make up the dense mesh of social processes used by health care professionals and patients, in making the health system malleable to the integration of their diverse cultures. In this study a community with global resources and a system with legitimated institutions, are made to face off as two equally complex social orders, with equally diverse cultural foundations. A fundamental equation is drawn between the resistance of the health care system to the processes of change emergent in the social trajectories defined by health consumers and that which becomes evident in the analysis of representations of health care by health care professionals and the positions they adopt in the articulation of that care. The construction of representations of knowledge by patients and the social forms they bring to bear on their actions is an invaluable analytic tool for identifying and interpreting the positions they take and how they are recognized and legitimized. The health system acts visibly, as a global system of resistance to the functioning of profane knowledge and to the diversity of social strategies that this knowledge gives rise to.
