Proposal of New Standard for Setting Doses of Steroid Ointments

Abstract

緒 言 アトピー性皮膚炎に対する薬物治療ではステロイド軟 膏による療法が広く用いられており 1) ，その治療効果には 軟膏の使用法・使用量が影響する 2) ．治療を行う際，患者 は自分自身で軟膏を適量取り出し患部に塗布することに なるが，ステロイドの塗布量は添付文書には「適量」と 示されているだけであり，定量的に明確な規定はなされ ていないのが現状である．また，患者は各種メディア等 から得られる情報をもとに独自の判断を行い，ステロイ ドの副作用を過剰に心配することもあり 3) ，これらの結果 としてステロイド軟膏製剤の適切な塗布量を正しく理解 しないままに使用し，患者自身の判断で必要以上に塗布 し，あるいは中止してしまう場合がある 4, 5) ．このよう に，ステロイド剤を用いた治療では患者がステロイド軟 膏製剤に関して正しい知識をもった上で治療を行うこと が望ましく，そのためのひとつの対策としてステロイド 軟膏製剤の塗布量に関する基準を明確に定めることが必 要である． 日本皮膚学会が発行するアトピー性皮膚炎診療ガイド ライン 6) にはステロイド軟膏製剤の塗布量の設定方法とし て Finger Tip Unit (FTU) と呼ばれる基準が推奨されてい る．FTU 基準は 1991 年に米国で初めて作成されたもので あり，軟膏の取出し口の口径を 0.5 cm，第 2 指の先端か ら第 1 関節部までの長さを 2.5 cm と固定した値を用いて いる 7, 8) ．1FTU に相当する軟膏製剤の量 (約 0.5 cm 3) は成 人の体表面積の約 2% 9) に相当する成人の掌 (手のひら) 2 枚 分に塗布するのに適しているとされ，FTU 基準に従い体 の各部位に薬剤を塗布する場合には，塗布する部位の体 表面積に対する割合を考慮して体の各部位における FTU に換算した場合の必要量を算出する方法 10­13) がある． この方法をそのまま日本国内で適用する場合，ステロ イド軟膏製剤の口径の大きさが製品によって異なる，患 者の体表面積や各部位の塗布面積の個人差については考 慮されていない，といった欠点もあり 14) 適切な指標とは いえない．特に，日本のステロイド軟膏製剤の口径は米 国と比べて小さい場合が多く，FTU 基準により得られる ステロイド軟膏製剤の塗布量に関する新規基準の提案 地嵜悠吾，中村暢彦 ＊ ，松村千佳子，矢野義孝 Regarding the dose amount of steroid ointments, package inserts gives no precise direction and it is necessary to clearly determine the standard method for dose setting. For this purpose, a method called 'Finger Tip Unit (F T U)' has been proposed in a guideline for atopic dermatitis, but this method has some demerits that it assume the same caliber of ointment tubes' head among different drugs' formulations and also it does not consider inter-patients' variability of body surface area. In order to establish a new dose setting strategy instead of F T U, we propose a new standard method which is based on the calculated surface area of body regions that drugs are applied. This method uses the published data of age, body weight, height, and surface area ratio of each body region to whole body surface area. Consequently, an equation to calculate the length of ointment to be squeezed out of a tube is presented for each body region, and we also propose the length for a unit area which is useful for applying to small area on any body regions. These results would be useful in the points that it clearly determine dose amount of steroid ointments and also it lead to an improvement of patients' adherence as well as improving clinical efficacy of steroid ointments.
