Aumento das taxas de suicídio no Brasil: o impacto da pandemia de COVID-19

Abstract

RESUMO Objetivo: Analisar a tendência das taxas de mortalidade por suicídio no Brasil nos períodos pré-pandêmico (2016–2019) e pandêmico (2020–2023). Métodos: Estudo ecológico de série temporal baseado nos óbitos por suicídio registrados no Sistema de Informações sobre Mortalidade (SIM). Foram incluídos óbitos classificados como lesões autoprovocadas intencionalmente (X60–X84), intoxicação exógena de intenção indeterminada (Y10–Y19) e sequelas correspondentes. As taxas por 100 mil habitantes foram calculadas por região, sexo, faixa etária e período. As tendências foram estimadas por meio de regressão de Joinpoint, com cálculo da variação percentual anual (VPA) e da variação percentual média anual (VPMA), ambas com IC95%. Resultados: No Brasil, houve tendência significativa de aumento no período total (VPA = 4,55%; p < 0,001). O pré-pandêmico apresentou crescimento significativo (VPMA = 5,07%; p = 0,03), enquanto o período pandêmico manteve tendência ascendente, porém menos intensa (VPMA = 3,87%; p = 0,021). Regionalmente, todas as macrorregiões apresentaram aumento no período total, com maior crescimento no Norte (VPA = 6,60%; p < 0,001) e Centro-Oeste (VPA = 5,27%; p < 0,001). Entre adultos (20–59 anos), observou-se aumento no período total (VPA = 4,87%; p < 0,001) e no período pandêmico (VPA = 6,26%; p = 0,038). Entre idosos (≥ 60 anos), houve crescimento no período total (VPA = 1,40%; p = 0,01). Homens e mulheres apresentaram tendência ascendente no período total, com aumento significativo durante a pandemia apenas entre homens. Na Região Sul, apenas Santa Catarina apresentou tendência crescente no período total. Conclusão: As taxas de mortalidade por suicídio no Brasil apresentaram tendência de crescimento antes e durante a pandemia, com variações regionais e demográficas importantes. A identificação de tendências diferenciadas entre regiões, faixas etárias e sexos reforça a necessidade de políticas de prevenção sensíveis às desigualdades populacionais e aos impactos de crises prolongadas sobre a saúde mental.ABSTRACT Objective: To analyze the trends in suicide mortality rates in Brazil during the pre-pandemic (2016–2019) and pandemic (2020–2023) periods. Temporal trends were estimated using Joinpoint regression, with annual percent change (APC) and average annual percent change (AAPC), both with 95% confidence intervals. Results: Brazil showed a significant increasing trend in suicide mortality over the entire period (APC = 4.55%; p < 0.001). Pre-pandemic years presented a significant increase (AAPC = 5.07%; p = 0.03), and the pandemic period maintained an upward trend of lower magnitude (AAPC = 3.87%; p = 0.021). All macro-regions exhibited significant increases in the total period, with the most pronounced growth observed in the North (APC = 6.60%; p < 0.001) and Center-West (APC = 5.27%; p < 0.001). Among adults (20–59 years), rates increased in the total period (APC = 4.87%; p < 0.001) and during the pandemic (APC = 6.26%; p = 0.038). Among older adults (≥ 60 years), a significant increase occurred in the total period (APC = 1.40%; p = 0.01). Men and women showed significant increases in the total period, but only men maintained a rising trend during the pandemic. In the Southern region, only Santa Catarina showed a significant increase in the total period. Conclusion: Suicide mortality in Brazil increased both before and during the COVID-19 pandemic, with notable regional and demographic disparities. These findings highlight the need for suicide prevention policies that are sensitive to population inequalities and to the prolonged social and health impacts of large-scale crises.
