Abstract

Objectives: 1. To introduce the Health Care Home conceptual model of patient centred care to Australia 2. To use the Quadruple Aim to generate data that demonstrates improvements in patient care 3. To describe two health reforms; voluntary patient enrolment and capitated funding for chronic disease. Method(s): A national trial of 65,000 patients, 200 General Practices and 10 Primary Health Networks educated through on-line educational modules and face to face train the trainer facilitation. Result(s): A cultural shift in the conventional wisdom model of delivery of patient centred care driven by education and training for clinicians and peak bodies. Conclusion(s): Primary Health Networks (10), General Practices (200) and Patients (65,000) have been enrolled in a national trial of the Health Care Home (HCH) in Australia. The HCH model is conceptually based on the Person Centred Medical Home predominant in the USA and the United Kingdom. It introduces two significant reforms to Australia: voluntary patient enrolment and capitated funding for patients with a chronic disease. Enrolled General Practices will use a software stratification tool to place chronic disease patients into one of three cohorts depending on the severity of their disease and comorbidities. Data using the Quadruple Aim (Outcome clinical indicators, patient experience, staff engagement and cost) will be used to evaluate the success of the trail and to determine which model of health reform will be introduced to the population as a whole. The presentation will cover all aspects of the HCH and the use of data to drive improvement.
