Intervención del maltrato hacia las personas mayores en el contexto familiar: una revisión sistemática

Abstract

Introducción: El maltrato geriátrico en el contexto familiar ha sido desatendido en comparación con otras formas de violencia intrafamiliar, se sabe poco sobre las formas más efectivas de intervenir este problema. Objetivo: Describir las características de los programas de intervención que reportan efectividad para el abordaje del maltrato geriátrico en el contexto familiar. Metodología: Este estudio teórico con metodología PRISMA. Se revisaron cuatro bases de datos: Scopus, Web of Science, Scielo y PubMed. Se aplica la prueba QUADAS II para evaluar la calidad de los estudios. Resultados: Se encuentran 9 programas de intervención para el abordaje de las diferentes formas de maltrato, 7 de 9 programas reportaron efectividad. Estos se caracterizan por ser en su mayoría ensayos clínicos aleatorizados con cegamiento y seguimiento. La mayor limitación se presenta en la alta probabilidad de sesgo por el flujo y tiempos de los estudios, pero reportan buenos indicadores de calidad. Discusión: Las características de los programas de intervención han mejorado a nivel metodológico, vinculan variables mediadoras como factores de riesgo (ansiedad, depresión, sobrecarga) y de protección (autoeficacia, capacidad de resolver problemas), pero pocos estudios realizan seguimiento de su impacto a largo plazo. Conclusiones: los programas efectivos se caracterizan por ser intervenciones con calidad metodológica, que vinculan variables mediadoras e implementan la educación y el entrenamiento en habilidades de afrontamiento, manejo del estrés, autocuidado, y habilidades de cuidado.Introduction. Abuse of older adults in the family context has received less attention than other forms of intrafamily violence, and little is known about the most effective ways to intervene in this problem. This study aims to describe the characteristics of intervention programs that report effectiveness in addressing abuse of older adults in the family context through a systematic review. Methodology. This is a theoretical study based on the PRISMA methodology. Four databases were searched: Scopus, Web of Science,Scielo, and PubMed. Rayyan software was used to organize and systematize the results. The QUADASII tool was applied to assess the quality of the studies. Results. Nine intervention programs aimed at addressing different forms of abuse were identified; seven of the nine programs reported effectiveness. Most were randomized clinical trials with blinding and follow-up. The main limitation was the high risk of bias related to study flow and timing; however, the studies reported good quality indicators. Discussion. The characteristics of the intervention programs have improved at the methodological level, incorporating mediating variables such as risk factors (anxiety, depression, caregiver burden) and protective factors (self-efficacy, problem-solving ability), nevertheless few studies conduct long-term follow-up of their impact. Conclusions. Effective programs are characterized by methodologically sound interventions that incorporate mediating variables and implement education and training in coping skills, stress management, self-care, and caregiving skills.
