Abstract

Background and objectiveAustralia’s health system faces challengesin the management and prevention ofchronic disease. Models of primary caredelivery, such as the Health Care Home(HCH) model, have been proposed to helpmeet these challenges. The aim of thisstudy was to explore pre-implementationconsumer perspectives of the HCH model.
