Abstract

Background: In January 2018 a Complex Discharge Unit (CDU) was developed with the overall aim to reduce the length of stay (LOS) of frail, acute patients, by providing optimal discharge planning, through multi-disciplinary team input. The #EndPJParalysis has demonstrated a 1.5 day reduction in LOS in acute settings (NHS, 2018), by having patients to be up and dressed by 12 pm. This #EndPJParalysis initiative was introduced in a CDU in March 2018. The overall aim of this study was to investigate the feasibility of this new initiative. Methods: Baseline satisfaction, current physical activity levels, current number of patients being up, dressed and active before 12 pm daily and any suggestions promoting physical activity were documented from all service users. Ward based exercise programmes, including classes and mobility status signs were prescribed by the physiotherapist. Staff, service user, family information sessions and information leaflets were given. Ward signage incorporating #EndPJParalysis were displayed. Ward based data was collected and analysed utilising Microsoft Excel and inputted via the #EndPJParalysis app. Results: 112 admissions to the CDU (Mean age 82.6 yrs), female (n = 66) have occurred since January 2018 with an average ward LOS of 22.1 days. 64% of patients have returned home with or without a home care package. All CDU staff attended #EndPJParalysis information sessions. All appropriate service users received exercise and mobility status programmes. The number of patients being up, dressed and active before 12 pm daily increased from 39% to 57% over the six week intervention period. Conclusion: The study demonstrated the feasibility of the #EndPJParalysis initiative in a CDU. Service users' and staff engagement were well received with a positive trend of increased numbers of patients being up, dressed and active by 12 pm.
