Clasp Retained Versus Attachment Retained Removable Partial Dentures for Treatment of Mandibular Distal Extension Cases: Patient Satisfaction and Oral Health-Related Quality of Life

Abstract

Objective: This study was carried out to assess patient satisfaction by visual analogue scale (VAS) and oral health-related quality of life (OHRQL) for digitally constructed clasp-retained and attachment-retained removable partial dentures (ARRPDs) in a mandibular class I partially edentulous cases Materials and methods: In this clinical trial,18 patients were equally divided into 2groups, they received mandibular class I computer-aided design/computer-aided manufacturing (CAD/CAM) constructed RPDs, patients were received RPDs retained with RPA clasp system (Group 1) for 6 months and the other patients were received RPDs retained with extra-coronal attachment (ARRPDs)) with cantilever for 6 months (Group 2). After receiving and wearing RPDs, patients were requested to submit a patient satisfaction questionnaire (VAS) and an oral health impact profile ( OHIP-14) questionnaire after 6 months with each denture. The Mann-Whitney test and the independent t-test, respectively, were used to compare patient satisfaction and OHRQL between groups. Result: Mann-Whitney test for OHIP-14 was done between the groups, and there was a statistically significant difference (p ≤0.001*) only in the life-satisfactory item. A nonsignificant difference was found between the groups in other items. Regarding the pronunciation of sounds (p =0.127 ), painful aching (p =0.495 ), feeling tense(p = 0.186), unsatisfactory diet (p =0.495 ), irritability with people (p = 0.127), Inability to function(p = 1.0 ) and Embarrassing (p =0.186). Independent t-test for comparing groups regarding (VAS) items showed a significant difference (p ≤0.001*) between groups for hygiene satisfaction, comfort with a prosthesis, esthetics, retention and stability, chewing efficiency, oral condition, and total satisfaction where it showed increased patients satisfaction with their prosthesis with ARRPDs. Except for the restoration of speech item, there was a nonsignificant difference(P=0.560) between the groups Conclusion: Within the constraints of this investigation. CAD/CAM constructed ARRPDs were recommended than the clasp retained RPDs as it demonstrated greater patient satisfaction and improved quality of life about oral health
