Infección de piel y tejidos blandos por Bacillus cereus

Abstract

RESUMEN El género Bacillus engloba a un grupo de bacterias altamente resistentes a condiciones extremas del ambiente. Bacillus cereus es un bacilo Gram negativo, móvil y flagelado, que se encuentra en la materia orgánica en descomposición, en el agua dulce y de mar, en algunos vegetales, en fómites y en el tracto intestinal de invertebrados, a partir de los cuales el suelo y los productos alimenticios pueden contaminarse. El espectro de infecciones causadas por Bacillus cereus incluye infecciones gastrointestinales, del tracto respiratorio, nosocomiales, oculares, del sistema nervioso central, cutáneas, endocarditis e infecciones del tracto urinario, y osteomielitis. Presentamos el caso de un paciente de 13 años con una infección grave de los tejidos blandos por Bacillus cereus, quien requirió tratamiento quirúrgico, antibióticos por vía endovenosa e, incluso, injerto de piel. Si bien Bacillus cereus no es un agente causal frecuente, debemos ser conscientes que su hallazgo tiene que valorarse con precisión, dada su potencial gravedad y la necesidad de adecuación del tratamiento antibiótico. Nivel de evidencia: IV ABSTRACT The genus Bacillus includes a group of bacteria that are highly resistant to extreme conditions in the environment. Bacillus cereus is a mobile and flagellated gram negative rod found in decaying organic matter, freshwater and sea water, plants, fomites and the intestinal tract of invertebrates from which soil and food products can be contaminated. The spectrum of diseases caused by Bacillus cereus includes infections of gastrointestinal tract, respiratory tract, nosocomial infections, ocular infections, central nervous system, skin, endocarditis, osteomyelitis and urinary tract infections. We present the case of a 13-year-old patient with severe skin and soft tissue infection due to Bacillus cereus who required surgical treatment, antibiotics administered intravenously and skin grafting. Although Bacillus cereus is not a frequent causative agent of cutaneous infection, we must have a high index of suspicion in certain circumstances, due to its high distribution in the environment and the potential seriousness of the infection it causes.
