Luxation tibio-astragalienne pure chez un jeune sportif

Abstract

We report the case of a 20-year-old athlete, who suffered a handball accident, causing a pure dislocation of the ankle (A). The reduction was performed under general anesthesia in emergency. X-Ray and CT scan after reduction objectified a good articular congruence (B, C). Restraint by a cast boot for six weeks was provided. The examination of the ankle performed after removal of the plaster has not found a laxity of the ankle. The ankle MRI performed in search of a ligament injury objectified integrity of periarticular ligaments (D). After nine months, the functional results were excellent, with no signs of instability or arthritis. The tibio-talar ankle dislocation without fracture associated is a very rare injury, often caused by high-velocity trauma; until now, few cases have been reported in the literature. The rarity of this lesion can be explained by the durability of ligaments compared with malleoli and thus, during a trauma of the ankle, a fracture occurs rather than a dislocation.
