Abstract
Background: Peripheral artery disease (PAD) is a global health burden with poor clinical outcomes in the aging society. PAD is considered a common manifestation of atherosclerosis and a risk factor for cardiovascular morbidity and mortality. It has been reported that PAD was associated with arterial stiffness and its peripheral artery obstructions can cause premature pulse wave reflections, which can lead to increase in augmentation index (AIx) and central blood pressure (CBP). Previous studies demonstrated that AIx and CBP were associated with cardiovascular disease mortality. However, the effect of endovascular revascularization on the arterial stiffness and central hemodynamics in patients with PAD remains unclear. Purpose(s): The aim of the present study was to assess the effect of endovascular therapy (EVT) on the arterial stiffness and central hemodynamics in patients with PAD. Method(s): This was a cross sectional study enrolling 55 patients with PAD. We measured augmentation index (AIx) and central blood pressure (CBP) using radial applanation tonometry before EVT and on the next day after EVT. Furthermore, we compared the therapeutic effects of EVT on AIx and CBP between iliac and superficial femoral artery (SFA) lesions. Result(s): Both AIx and CBP were significantly improved after EVT (85+/-17% to 74+/-18%; P<0.001 and 125+/-22 mmHg to 111+/-21 mmHg; P<0.001, respectively). In patients with iliac lesions, both AIx and CBP were significantly improved after EVT (89+/-15% to 76+/-15%; P<0.001 and 124+/-18 mmHg to 111+/-14 mmHg; P<0.001, respectively). Similarly, in patients with SFA lesions, both AIx and CBP were significantly improved after EVT (82+/-18% to 73+/-20%; P<0.001 and 125+/-25 mmHg to 110+/-25 mmHg; P<0.001, respectively). Endovascular revascularization reduced both AIx and CBP, regardless of whether the target lesion is iliac or SFA. Conclusion(s): Both AIx and CBP were significantly improved after EVT and these effects were similar to those in both iliac and SFA lesions, indicating that EVT could have an effect of improving cardiovascular disease in patients with PAD.
