Çocuklarda Epilepsi Nöbetlerinin Hastane Dışında Yönetilmesine Yönelik Verilen Eğitimlerin Etkinliğinin Değerlendirilmesi: Randomize Kontrollü Çalışmaların Sistematik Derlemesi

Abstract

Objectives: To evaluate the effects of the education program for the management of epilepsy seizures out of the hospital in children who have epilepsy. Methods: Screening was conducted between April-May 2019 at Akdeniz University's electronic databases; CINAHL, Cochrane Library, Sciencedirect, WOS, Wiley, Medline, Ulakbim, Pubmed, YÖK National Thesis Center electronic databases were scanned with the keywords "children", "epilepsy", "prehospital seizure management", "home management of seizure" without year limitation. Results: All in all, 1623 studies were reached. 421 was duplication. Out of the remaining 1399 studies, 180 studies were reached that met the inclusion criteria concerning language-title-summary. Three randomized controlled trials were included in the full text of this study. The first trial aims to increase the first aid knowledge of the parents, the second trial aims to increase the general knowledge and skill level of the parents, the third trial aims to strengthen the medical management of the parents at the time of the seizure. It was stated that education programs for the management of epilepsy seizures were effective in three studies. Conclusion: It is possible to teach the caregivers to the caregivers with correct and timely interventions outside the hospital while eliminating the possible negative situations and improving the quality of life of the family. However, this issue should be studied more with larger groups and with a high level of evidence, and nurses should shed light on education for seizure management to caregivers outside the hospital. Gereç ve Yöntem: Tarama Nisan-Mayıs 2019 tarihleri arasında, Akdeniz Üniversitesi elektronik veri tabanları CINAHL, Cochrane Library, Sciencedirect, WOS, Wiley, Medline, Ulakbim, Pubmed, YÖK Ulusal Tez Merkezi'nden; "çocuk", "epilepsi", "hastane dışı nöbet yönetimi", "evde nöbet yönetimi" ile buna karşılık gelen ingilizce anahtar kelimeler ve MeSH terimleri ile yıl sınırlaması olmaksızın gerçekleştirilmiştir. Bulgular: Taramalarla toplam 1623 çalışmaya ulaşılmıştır. 421'i dublikasyondur. Kalan 1399 çalışmadan, dil-başlık-özet bakımından dahil etme kriterlerine uyan 180 çalışmaya ulaşılmıştır. İçerik, çalışma tipi uygun olan, tam metnine ulaşılan üç randomize kontrollü çalışma dahil edilmiştir. Çalışmalardan ilkinin ebeveynlerin nöbet anı ilkyardım bilgilerini artırmayı, ikincisinin ebeveynlerin nöbet anına yönelik genel bilgi ve beceri düzeyini artırmayı, üçüncüsünün ebeveynlerin nöbet anı medikal yönetimi güçlendirmeyi hedeflediği görülmektedir. Üç çalışmada, çocuklarda epilepsi nöbetlerinin hastane dışında yönetilmesine yönelik verilen eğitimlerin, nöbet yönetiminde etkin olduğu bulunmuştur. Sonuç: Nöbetin hastane dışında doğru ve zamanında müdahaleler ile bakım verenlere öğretilmesi hatalı yaklaşımları ortadan kaldırırken, ailenin yaşam kalitesini artırmaktadır. Bu konuda büyük gruplarla kanıt düzeyi yüksek araştırmalar yapılmalı, hemşireler bakım verenlere hastane dışında nöbet yönetimini öğreterek alana ışık tutmalıdır.
