Abstract
The most recent and controversial ideas with regard to total knee replacement (TKR) are reviewed. It must be emphasized that the results of TKR are generally highly satisfactory and this technique provides an acceptable solution to many degenerative knee processes. TKR eliminates or alleviates preoperative pain and improves the function and quality of life of the patient for a considerable time period. Low-friction systems, mobile platforms, non-cemented systems, and substitution of the patella and posterior cruciate ligament (PCL) are topics for which the medical evidence is currently inconclusive, until more long-term studies are carried out. Meticulous surgical technique and adequate patient selection are probably more important than design factors in the outcome.
