Abstract
The National Quality Infrastructure (NQI) in a given country provides the essential framework for quality assurance integrating standardization, metrology, accreditation, and conformity assessment to create the technical backbone for industry and trade. Hence, building a resilient NQI is fundamental to securing medicine quality, enabling local production, and enhancing resilience against supply chain disruptions. This archival review aimed to evaluate Ethiopia's quality infrastructure in relation to that of South Africa, Rwanda, and the Republic of Korea, with the aim of identifying existing gaps and opportunities in services provision to strengthen local pharmaceutical manufacturing capacity and foster regional integration in the pharmaceutical market. A structured archival review tool was used to analyze and compare the legal frameworks and practical implementation of National Quality Infrastructure (NQI) in four selected countries. Ethiopia, South Africa, Rwanda, and the Republic of Korea were selected based on their standing in the Global Quality Infrastructure Index and their World Health Organization (WHO) maturity levels in medicines regulation. The review focused on assessing how these NQI systems support pharmaceutical manufacturing. The comparative analysis showed significant disparities in quality infrastructure development across the four selected countries. Ethiopia lags behind South Korea and South Africa, both of which have well-established systems with comprehensive infrastructures, including availability of reference materials for calibration and accredited pharmaceutical testing laboratories. In Ethiopia, the Metrology Institute (EMI) provides only limited calibration services, forcing reliance on costly foreign calibration that increases production expenses and delays. The country also lacks a coherent national quality policy, faces fragmented governance, and has not developed a strategic, pharmaceutical-focused NQI. In contrast, Rwanda has aligned its standards with the East African Community, enhancing domestic pharmaceutical production and regional market access. Furthermore, the absence of ISO/IEC 17025 accreditation among local manufacturers in Ethiopia undermines their ability to demonstrate product quality to regional regulators, limiting market entry opportunities, an area where Rwanda holds a comparative advantage. Strategic enhancements in Ethiopia's quality infrastructure, including alignment with regional standards, expanded metrological calibration for pharmaceuticals, and sector-specific accreditation programs, are critical to transitioning from import reliance to regional export competitiveness. Realizing this shift necessitates policy integration of quality infrastructure reforms with national pharmaceutical strategies and regional trade frameworks, ensuring coherence between regulatory capacity, industrial development, and market integration.
