Abstract
Introduction: Although the use of statin is mandatory in patients with acute myocardial infarction (AMI), it has been suggested to be associated with new onset diabetes mellitus (NODM). In real world practice, moderate intensity statin is more commonly used than high intensity statin. Purpose: In this study, we investigate the impact of moderate intensity pitavastatin (2 mg of 4 mg) on the development of NODM compared to atorvastatin (10 mg to 20mg) up to three years follow up. Methods: From November 2011 to May 2015, we investigated a total of 2001 AMI patients who did not have DM. To adjust potential confounders, a propensity score matching (PSM) analysis was performed using the logistic regression model. After PSM, a total of 480 patients (pitavastatin group, n=240 patients and atorvastatin group, n=240 patients) were enrolled for analysis. Results: Before PSM, the cumulative incidence of NODM is lower in pitavastatin group than atorvastatin group (2.6% vs. 7.8%, p-value = 0.005). Even after PSM, the pitavastatin still had a significantly lower incidence of NODM than atorvastatin (2.3% vs. 8.6%, p-value = 0.005). The incidence of MACE (10.9% vs. 8.8%, pvalue = 0.423 before PSM and 11.7% vs. 10.4%, p-value = 0.800 after PSM) and MACCE (11.3% vs. 9.9%, p-value = 0.609 before PSM, 12.1% vs. 11.0%, p-value = 0.792 after PSM) was similar between the two groups. Conclusions: Among the patients with AMI, moderate intensity pitavastatin was associated with lower incidence of NODM and similar clinical outcome compared to moderate intensity atorvastatin. Randomized controlled trials will be needed to get the final conclusion. (Table presented).
