Abstract

L-transposition of the great arteries is a complex cardiac malformation with ventricular inversion and discordance of both atrioventricular and ventriculoarterial connections. Several electrophysiologic abnormalities can be associated with this entity including varying degrees of heart block, supraventricular arrhythmias such as sick sinus syndrome and supraventricular tachycardias (atrial flutter, atrioventricular reentrant tachycardia), as well as ventricular tachycardias. A review of the anatomic and functional abnormalities of the conduction system in L-transposition is provided along with a summary of the unique problems associated with the use of pacemakers in this patient population.
