Correction of malunion following distal radial fractures requires a sound knowledge of the fracture mechanism, in addition to which the anatomy of the fracture site and the risks and potential benefits of conservative and operative treatments need to be considered thoroughly. Treatment aims at anatomically correct restoration of the joint surface and realignment of the radial axis to reduce the risk of posttraumatic arthrosis. We report on the diagnosis of malunion and the indications for and typical surgical options available for realignment osteotomy, with special reference to planning, complications and postoperative physiotherapy in the case of posttraumatic malalignment in the distal radius. In adult patients, axis deviation of more than 20° in either plane should be corrected, as should radial shortening by more than 2 mm. © Springer Medizin Verlag 2006.
