Abstract
In the originally published version of this article, there was a dosing unit error on page 375. The unit should read “mg/kg/d” instead of “mg/kg” in the following passage: “Based on published data, doses of 60 to 80 mg/kg administered every 6 hours are required to achieve this goal AUC in children 3 months to 12 years of age with normal renal function. Doses of 60 to 70 mg/kg administered every 6 or 8 hours are recommended for patients age 12 years or older.” This error has been corrected online.
