This position statement addresses the most relevant issues pertaining to a comprehensive disease management program (DMP) for patients with chronic heart failure (CHF). 1. Why is a DMP needed? 2. How should a program be organized and how should it function? 3. Which professional groups should be involved? 4. Which patients should be included? 5. What should be the duration of patient care in the program? 6. What are the costs and cost-effectiveness?
