Noninvasive positive pressure ventilation treatment with helmet and face mask in a case with bilateral bullous lung and acute respiratory insufficiency

Abstract

Noninvasive positive pressure ventilation (NPPV) is a respiratory support method which is increasingly used for acute and chronic conditions as well its use is getting more frequent. A 67-year-old male followed up for chronic obstructive pulmoner disease in chest disease department in our hospital, was taken into respiratory intensive care unit since his arterial gas analysis revealed that PaO2 was 28 mmHg, Pa CO2 was 76 mmHg and pH was 7.25. Giant bullae were detected in his both lungs on chest radiograms. Intermittent NPPV was applied with a face mask on ventilation device. Since laceration and bleeding mask were developed on the 3rd day due to face mask, NPPV was administrated with a helmet at the same pressures. The case whose respiratory parameters were improved, was transferred to chest diseases service on the ninth day. NPPV management can be successful in carefully selected patients with a close follow-up. © 2010 by Tür ki ye Kli nik le ri.
