P2509Associations of emotional distress in patients with first myocardial infarction. Results from SWEDEHEART registry

Abstract

Background: Factors associated with emotional distress (ED) in a real world, large, national registry of MI patients are not well known. Purpose(s): To evaluate factors associated with ED in first time MI patients <75 years. Method(s): Data from the national SWEDEHEART registry on 27,267 consecutive patients with a recent MI with a follow-up at 2 and 12 months post-MI (n=22,911) were used. ED was assessed with the EQ-5D questionnaire at both time points. In multivariable models, factors assosciated with ED, OR (95% CI) were assessed. Demographic variables, previous medical history, symptoms on admission, inhospital treatments, complications, and duration of hospital stay were evaluated. Result(s): Prevalence of some level of ED was frequently reported (38% and 33%) at 2 and 12 months post-MI. Strong factors associated with ED (Table) at 2 months were female sex, age, country of birth and employment status. The strongest association was a prior depression or anxiety and readmission due to CV events. Clinical factors were less strongly associated with ED. At 12 months post-MI, the same variables were associated with ED and with similar effect sizes. Clinical variables (LVEF, cardiac arrest, Killip class and AF) were not associated with ED. Conclusion(s): Strong factors associated with ED were previous depression or anxiety, demographic variables, and readmission. In contrast, medical history and other clinical variables seemed less important. (Table Presented).
