In this review, we survey some significant advances in the medical care of babies <1000g and we highlight the development of care pathways that ensure optimal antenatal care, which is a prerequisite for good neonatal outcomes. We also suggest that the long overdue development of family integrated care will in the end prove at least as important as the recent medical advances.
