Abstract
Chronic diseases are a significant burden on healthcare systems worldwide, requiring ongoing management and care to prevent complications and improve quality of life for patients. Nurse-led interventions have been shown to have a positive impact on chronic disease management, helping patients adhere to treatment plans, make positive lifestyle changes, and improve their overall health outcomes.
