Abstract
Best practice management of chronic heart failure involves multidisciplinary care. Home medicines reviews are an important and yet under-used service for people living with heart failure. There is strong evidence that HMRs can reduce readmissions to hospital for heart failure, and improve adherence to lifestyle and complex medication regimens. Assessment of adherence to long-term therapies is the first step to improvement. As many patients will have multiple other conditions, the risks and benefits of therapy need to be assessed against current and emerging evidence.
