Abstract

332 CAT-SCRATCH DISEASE WITH SYSTEMIC ENVOLVEMENT, OSTEOVERTEBRAL OSTEOMYELITIS AND PARAVERTEBRAL ABSCESS Cat-scratch disease (CSD) is a relatively common acute and benign infectious disease caused by Bartonella henselae, with only a small proportion of patients showing extranodal manifestations. We present two patients, a 9-year old girl and a 10-year old boy with CSD complicated by osteomyelitic envolvement of vertebra, sternon, skull and pelvis, paravertebral abscess, splenic enlargement and lymphadenopathy in one case, and severe vertebral envolvement with a destructive process in the other. Both were treated with clarithromycin (in the first case rifampin was added), with a favorable clinical outcome and recovery in their radiological and scintigraphic images. We discuss the clinical features and management of both cases according to a review of previously published cases. INTRODUCCION La enfermedad por arañazo de gato (EAG) es una frecuente causa de adenopatía, en general benigna y autolimitada. Ocurre predominan-temente en la infancia y en el adulto joven. Se ha demostrado que es causada principalmente por un pequeño bacilo pleomórfico Gram nega-tivo, Bartonella henselae. 1 Se han descrito cuatro criterios que permiten establecer el diag-nóstico: • la existencia de un gato, frecuentemente jo-ven, con o sin un sitio de inoculación en el paciente. • linfoadenopatías, en general vecinas a la le-sión de arañazo, con pruebas de laboratorio que permitan descartar otras etiologías. • test intradérmico diagnóstico o actualmente test serológico, positivos para B. henselae. • histología característica: bacilos teñidos con tinción argéntica de Warthin-Starry y linfadenitis granulomatosa supurada. 2
