Abstract

L a obesidad, "epidemia del siglo XXI" según la Organización Mundial de la Salud, es uno de los mayores y más desatendidos proble-mas de salud pública de nuestros tiempos. 1 La obe-sidad y el exceso de grasa visceral predisponen a hipertensión arterial, hiperglucemia y trastornos lipídicos, identifi cados como síndrome metabólico (SM). 2 El SM se asocia con al menos el doble riesgo de presentar enfermedad cardiovascular 3-5 y diabetes mellitus tipo 2. 6 En los trabajadores de la salud, la prevalencia de obesidad suele ser menor que en la población general, pero al menos 10 % de estos trabajadores desarrollarán síndrome metabólico 7-9 o enfermedad cardiovascular dentro de los siguientes 10 años. 10-11 En un estudio colombiano en trabajadores de la salud, 21 % presentó obesidad, 46.4 % sobrepeso y 13.2 % síndrome metabólico, según los criterios NCEP-ATP III. 12 En México, y de acuerdo a la ENSANUT 2012, la prevalencia combinada sobre-peso-obesidad es mayor en mujeres (73 %) que en hombres (69.4 %) y también de obesidad (37.5 % hombres frente al 26.8 % mujeres). México está inmerso en un proceso de aumento inusitado en la prevalencia de obesidad y se encuentra entre los países en los que más rápido se ha desarrollado esta enfermedad, donde 7 de cada 10 adultos presentan sobrepeso y de estos, la mitad presentan obesidad. De 1988 a 2012 el sobrepeso en mujeres de 20 a 49 años de edad se incrementó de 25 a 35.3 % y la obesidad pasó de 9.5 a 35.2 %. 13 En estudios previos a personal hospitalario mexi-cano, se documentó prevalencia de 15-40 % de sobre-peso y 20-31 % de obesidad. 14-17 Sobrepeso, obesidad, síndrome metabólico e índice cintura/talla en el personal de salud Overweigth, obesity, metabolic syndrome and waist/height index in health staff Introduction: Health staff self-applied strategies are insufficiently to reduce cardiovascular risk factors. The aim of this article is to investigate the prevalence of overweight, obesity and metabolic syndrome and waist/height index on health staff. Methods: This is a cross-sectional study from January to April 2014 in 735 workers of the UMAE Specialties La Raza. A diagnosis of overweight and obesity was established according to WHO criteria and metabolic syndrome NCEP ATP-III. In 250 participants lipid pro-fi le and blood chemistry were obtained. Descriptive statistics was applied, chi square, Student t test or Kruskal-Wallis to compare groups. Results: We included 496 women and 239 men (physicians, nurses and other workers). 38 % were overweight; 22 % had obesity. We found fasting hyperglycemia > 126 mg/dl in 5.1 %, between 100-126 mg/dl in 17.4 %, hypertriglyceridemia in 40.4 %; HDL cholesterol < 40 mg/dl 66 % in men and HDL cholesterol < 50 mg/dl 51.4 % in women. Waist measurement was 93 ± 11 cm in men vs. 88 ± 13 m in women. The waist/ height index in the ideal weight group was 0.048 ± 0.04, for overweight 0.55 ± 0.04 and for obesity 0.65 ± 0.064 (p < 0.01). Metabolic syndrome prevalence was 30.6 %, smoking 21.6 % and alcohol consumption 35 %. Conclusions: The prevalence of obesity-overweight was 60 % and metabolic syndrome 30 %, making it necessary to take immediate actions to modify the lifestyle of health staff.
