A patient with acute renal failure due to hypercalcemia associated with activated vitamin D3 ointment, who had undergone hemodialysis

Abstract

Key words：activated vitamin D3 ointment, hypercalcemia, acute renal failure, hemodialysis 〈Abstract〉 A man in his 60s had been treated for psoriasis vulgaris with activated vitamin D3（maxacalcitol）ointment and etretinate since 2006. In August 2010, he developed diarrhea, anorexia, and general fatigue. Under a diagnosis of viral enterocolitis, he underwent parenteral fluid therapy by a local physician. However, general fatigue persisted despite improving his diarrhea；therefore, he was admitted to our hospital. Since the serum creatinine level was ／ ／ 11.16 mg／dL and potassium was at 8.1 mEq／L with metabolic acidosis, hemodialysis was initiated. As he had been treated with maxacalcitol ointment for long periods, we therefore susupected acute renal failure due to hypercalcemia resulting from the maxacalcitol ointment, and so discontinued it. After five hemodialysis sessions ／ and conservative management, his renal function improved to a serum creatinine level of 2.46 mg／dL. Finally, this patient was discharged from our hospital with mild renal dysfunction. We should always consider that activated vitamin D3 ointment may result in hypercalcemia and acute renal failure. 平山 尚 福山市民病院内科 〒 721-8511 広島県福山市蔵王町 5-23-1 Takashi Hirayama Tel：084-941-5151 Fax：084-941-5159 〔受付日：2011 年 8 月 2 日，受理日：2011 年 9 月 27 日〕 症例は 60 歳代，男性．2006 年 4 月より尋常性乾癬に対し，合成レチノイド製剤（エトレチナート）内服と活性型 ビタミン D3（以下 VD3）外用剤（マキサカルシトール）を併用していた．2010 年 8 月下旬より水様性下痢，食欲不 振が出現し，ウイルス性腸炎の診断で近医で保存的加療を行われていた．その後，下痢症状は改善するも全身身怠 ／ 感が持続するため，近医より当科に紹介受診となった．来院時，尿素窒素（BUN）158.2 mg／dL，血清クレアチニン ／ ／ ／ （Cr）11.16 mg／dL，カリウム（K）8.1 mEq／L，重炭酸イオン（HCO3 − ）15.6 mmol／L，補正カルシウム（Ca）12.4 ／ mg／dL と急性腎不全，高 K 血症，代謝性アシドーシス，高 Ca 血症を認めたため入院，緊急血液透析を施行した． 長期にわたり VD3外用剤を使用されていたことから，VD3外用剤による高 Ca 血症とそれに伴う急性腎不全と推察 された．VD3外用剤と合成レチノイド製剤内服を中止し，計 5 回血液透析を実施後に離脱した．その後，腎機能は ／ Cr 2.46 mg／dL まで改善し第 58 病日に退院となった．VD3外用剤の副作用として，高 Ca 血症と腎機能障害の報告 例はいくつか認めるが，その多くは外用剤の中止と高 Ca 血症に対する治療で軽快しており，緊急血液透析を要し た報告はない．今回，われわれは VD3外用剤の副作用によって，緊急血液透析を要した 1 例を経験したので報告す る．
