In the PDF version of this Article originally published, authors Ana da Silva Filipe, James G. Shepherd, Thomas Williams and Joseph Hughes were mistakenly affiliated with the London School of Hygiene and Tropical Medicine; instead, they should have been listed as equally contributing authors. Also, Matthew T. G. Holden, David L. Robertson, Kate Templeton and Emma C. Thomson were incorrectly listed as equally contributing authors; they should have been jointly supervising authors.
