Abstract

The documentation of health education is an essential process in nursing practice, recording the educational information provided to patients. It serves as legal proof for nurses and supports the integration of tasks among them. The completeness of this documentation is driven by nurses' motivation and perception of its importance in improving the quality of care. This study aims to determine the relationship between nurses' motivation and perception with the implementation of health education documentation at Arifin Achmad Regional General Hospital, Riau Province. The research employs a quantitative study with a descriptive-correlational design using a cross-sectional approach. The sample consists of 47 nurses selected through a total sampling technique. The research instruments include questionnaires and observation sheets. Data analysis was conducted using the chi-square statistical test. The results showed that 24 nurses (51.1%) had high motivation, while 23 nurses (48.9%) had low motivation. Additionally, 27 nurses (57.4%) had a good perception, whereas 20 nurses (42.6%) had a less favorable perception. The chi-square statistical test results indicated that nurses' motivation had a p-value of 0.627, and nurses' perception had a p-value of 0.689, where the p-value was> 0.05. This means there is no significant relationship between nurses' motivation and perception of the implementation of health education documentation. It is recommended that the head of the ward and the hospital provide recognition or rewards to nurses who actively document health education. Furthermore, nurses should consistently adhere to Standard Operating Procedures (SOPs) to ensure greater attention to every aspect of health education documentation during the documentation process.
