Abstract
Background/Introduction: Aortopathies related to Takayasu arteritis (TA), Marfan syndrome (MS) and other Marfan-like syndromes are characterized by dilation of the aortic root, and its main cause of mortality is the dissection and rupture of the aorta. The measurement of vascular stiffness parameters by echocardiogram, MRI or tonometry and their association with the progression of aortic dilation has recently been reported. In this study we evaluated the use of the Arteriograph in this patients. Purpose: To evaluate the measurement of central blood pressure (CBP), augmentation index (AI) and aortic pulse wave velocity (PWVAo) in patients with aortopathies. Methods: Cross-sectional observational study including 18 patients with genetic aortopathies (2 MASS syndrome, 2 Loeys-Dietz syndrome, 2 Ehlenrs Danlos syndrome and 12 MS), 4 subjects with TA and 44 healthy controls matched by age and sex. All diagnosis were based in Ghent or ACR criteria. The variables are reported in median and 25-75 percentiles. Results: The age of the subjects is higher in subjects with TA. The female sex was predominant. Aortic root dilation was observed only in two subjects with TA. CBP is similar in controls and genetic aortopathies, but higher in TA. A positive gradient was observed between brachial systolic pressure and CBP in controls and genetic groups, but negative in the TA. The AI is higher in the TA, even after indexing it to body surface. The PWVAo is higher in the TA, without reaching statistical significance (see table). Conclusion(s): Patients with genetic aortopathies without aortic dilation still present CBP, AI and PWVAo similar to controls. Subjects with TA have higher CBP and AI than controls. The non-invasive measurement of these parameters could potentially be useful in the diagnosis and follow-up of these patients. (Table Presented).
