Abstract
Objective: To describe the distortions of therapeutic milieu in psychotherapy of incest-victim patients. Method: We describe a case-series of 59 women victims of incest, and we summarize the current bibliography on the issue of transference and countertransference in the care of these patients. Results: There is paucity of research on the issue of transference/countertransference on these patients. We identified four main types of counter transference (CT), which are not exclusive: Vicarious CT in which the therapist assumes a negative attitude of absent parents, gives affection, and is always prone to abate anxieties, fear and guilt feelings. Super-Ego CT, addressed to punish the sexual aggressor, forgetting at times the needs and claims of the victim. Negative mothering CT, in which the therapist blames the victim for the family disruption. Negative parental CT, in which the father's figure is seen as absent, threatening, seductive, or careless of his daughter's needs. The gender of the therapist and the therapeutic approach influence the approach to these patients. © 2007 Sociedad de Neurología, Psiquiatría y Neurocirugía.
