A case of parathyroid carcinoma invading the trachea and superior mediastium

Abstract

Parathyroid carcinoma is relatively rare. A 48-year-old male was referred to the hospital complaining of a front neck tumor. Multiple tumor lesions were detected in the chest X-P. He was then examined at our hospital. Hyperparathyroidism was suspected from the laboratory data including hypercalcemia and a high PTH level. A neck CT showed a right frontal neck mass invading the trachea and superior mediastium. A chest CT showed multiple pulmonary metastases. He underwent en bloc resection of the parathyroid carcinoma with part of the trachea. After surgery, he was treated with chemotherapy and radiation for multiple pulmonary metastases. The calcemia and PTH level dropped for a while, but gradually increased. An MRI showed brain metastasis and he was treated with γ-knife. He underwent resection of the pulmonary metastases and was treated with alcohol injection therapy for multiple metastases. At present, the calcemia and PTH are slightly high, and he has been followed up without symptoms.
